DISCUSSION:
The purpose of this memorandum is to introduce an updated version of the Transitional MO HealthNet Quarterly Reports. All fields are now fillable. The fields containing the participants’ name and address and the Family Support Division office address have been revised, and the FSD INFO telephone number added. These forms are located in the forms manual.
