DISCUSSION:
The Applicant’s Eligibility Statement (FS-1) form, English and Spanish translations, are revised. A statement is added to notify the eligibility unit that a "yes" response to any of the Household Declaration questions may result in a disqualification from the Food Stamp Program for that individual. The EU is also informed that reporting the Hispanic/Latino ethnicity of an individual is voluntary and does not affect the individual’s eligibility or benefit amount for the Food Stamp Program. The FS-1 instructions are updated and revised.
The new FS-1 forms and instructions will be available on the Intranet and Internet at a later date. The PDF version may be printed and given to anyone requesting an application for food stamp benefits.
To access the PDF version and/or instructions,
- go to Family Support Division (FSD) Intranet site,
- select Food Stamps,
- under Forms select Forms Manual from the drop-down list,
- select Volume 1, and
- select PDF FS-1 English, FS-1 Spanish or Instructions.
The new FS-1 forms and the instructions will be available at a later date on the Department of Social Services Internet site at http://dss.mo.gov/fsd/fstamp/.
An initial supply of the revised FS-1 is being sent to the county offices. Upon receipt, destroy/recycle all versions prior to 09/06.
