INSTRUCTIONS FOR MAKING YOUR FOOD STAMP APPLICATION & FOOD STAMP RIGHTS

PURPOSE: To provide information to the applicant regarding:

• his/her rights;
• interview requirements;
• when the EU can expect to receive Food Stamp benefits;
• voluntarily providing race/sex (including Hispanic);
• immigration status;
• Social Security Number (SSN);
• time limits;
• filing joint applications; and
• instructions for completing the Food Stamp application, etc.

NUMBER OF COPIES AND DISPOSITION: One copy is sent to the EU with the FS-1 (English) and the system generated Notice of Expiration (NOE). The Spanish translation is provided by the county office upon request and when the Spanish FS-1 Food Stamp application is completed.

If an individual wishes to download the IM-31F from the Internet, instruct the individual to go to http://dss.mo.gov/fsd/food-assistance/food-stamp-program/. Both the English and Spanish translations are on the website.
