CLIENT SERVICES POSTCARD

PURPOSE: To provide the clients of the Family Support Division with a method to give feedback on services received.

NUMBER OF COPIES AND DISPOSITION: Copies are to be placed in each county’s reception office in such a way that they are easily and readily available for use by clients. Completed copies should be dropped in the mail by the client. The completed copy will be mailed to the Division Director’s office. Quality Assurance tabulates the feedback and produces summaries. Summary data from the feedback will be used in Quality Assurance reviews.

INSTRUCTIONS FOR COMPLETION:  The postcard is to be completed by the FSD client. Name, address, and telephone number are optional data. Any completed postcards left by the client in the county office must be forwarded to the Income Maintenance Quality Assurance Unit in State Office.

NOTE: County staff must ensure that postcards are available to the public and that the county information (office location) is entered on the card prior to making them available to the customers.
